Please fill in the forms below with as much detail as possible. The information you provide will be used for:<\/p>\n<ul>\n<li>Registering the death with Births, Deaths, and Marriages<\/li>\n<li>Completing the Cremation Application<\/li>\n<li>Completing the Authorisation for Cremation and the Financial Agreement.<\/li>\n<\/ul>\n<p>You will need to set aside about 15 minutes to complete this form. An email address for someone to witness this form will also be required.
